What is in the Provider Directory?

This directory is comprised of County and Contracted Providers within our behavioral health services network. It includes the following:

Practitioner names and individual practitioners within those providers
Physical locations
Telephone numbers
Type of services provided and populations served such as children, youth, or adults
If practitioners is accepting new members
Cultural capabilities
Languages offered
Type of individual practitioners such as a Licensed Clinical Social Worker or Licensed Professional Clinical Counselor

Services may be delivered by an individual practitioner, or a team of practitioner, who is working under the direction of a licensed practitioner operating within their scope of practice. Only licensed, waivered, or registered mental health providers and licensed substance use disorder services providers are listed on the Plan’s provider directory.

DMC County members have a right to obtain covered services from any enrolled and DMC-certified provider, even if that provider is not listed in the provider directory.

How do I search the Provider Directory?

Select a service type from the drop-down.
Select the information you would like to see (Provider or Practitioner) from the drop-down.
Click “Search”.
This allows you to see all the information available related to the selected service type and provider or practitioner information.
To further narrow down your search, you can select additional information in the sidebar such as name, provider, language, distance from the San Jose city center, nearby location to a specific address, and if you would like the information sorted alphabetically (A-Z or Z-) or distance.
Once you have made the additional selections, click “Search” and you will see the information related to your narrowed down search.
If you would like to change your search but not change the service type, click “Reset” and it will refresh the page.
If you would like to change your search to a different service type, click “Change service type” and the page will be refreshed back to the main page.

How do I view the information of a Practitioner?

Click on the name of a practitioner and information details will appear for that practitioner. From this window, you can also share the information by email, print, map the location from an address, and find more information on the individual practitioners at that provider.
